Introduction
Case Managers offer assistance and support to youth through age 18 and families who are dealing with issues surrounding a mental health disorder and/or who are identified as "at risk" for Serious Emotional Disturbance (SED).
Services Provided
Case Management services include:
- Development and monitoring of Individual Service Plan to address the youth and family goals.
- Identification of community resources and supports to assist the youth and the family. The case manager will assist the family in accessing these services. Examples of services may include: Individual/Group/Family counseling; Psychiatric treatment; Medical treatment; Mentoring; Tutoring; Daycare; Therapeutic Recreation; Health Insurance; Support Groups; Parenting Education; etc.
- Advocacy and liaison with external agencies including the schools, courts, and community assessment teams.
- Assistance with emergent issues as needed which may include transportation, housing, and medication.
An initial intake assessment will be completed to determine eligibility for services. Please call to schedule your child’s intake session. Plan to spend at least 90 minutes completing intake. Legal guardian and child must both be present.
